Description

Nurse Practitioner needed in New York for Adult Medicine coverage. Hybrid role Telehealth and Home Health located in NYC any borough. Must be Board Certified, have current DEA, and NY state license. Shifts will be Monday - Friday 8 am - 5 pm. Will see 12-15 patients per day for telehealth and about 6 patients per day for HH. Adult patient population is geriatrics. 3 years experience in adult primary care required. Support staff includes MA's, RN's, scribes, behavioral specialists, physicians, practice manager and admin staff. Provider will conduct The schedule will be M-F 8-5, and they'll be visiting about 6 patients a day for HH visits. 2-3 days telehealth and 2-3 days home health per week. Provider will perform home health visits in private residence with an MA and a scribe.
